Establish a Household ID
Establish your
household number and family member numbers by
e-mailing our offices with the
following information:
- name of the guardian or parent or primary member (please include first name, last name,
date of birth and gender)
- address (please include city, state, zip and any unit or apartment number)
- home phone number (including area code)
- emergency numbers (including area code)
- all the family members who will be included (please include first name, last name, date of
birth and gender for each individual)
