What is CHIP?
CHIP is the Claims and Health Information Portal, an internet tool
implemented by the City of Mesa Employee Benefits office. By using CHIP,
you can view your health benefits information 24 hours a day, 7 days a week, 365
days a year.
CHIP lets you:
- View who is covered under your medical, dental, and/or vision plans;
- View your summary of plan benefits;
- Check individual and/or family deductibles;
- Check if your child's student status is current;
- View your claim history and charge information;
- Link to related sites such as EAP Preferred, Blue Cross Blue Shield,
HealthSmart, Medco,
and Avesis.
Privacy and Security
The City of Mesa Employee Benefits Office treats your personal information
securely and confidentially. We limit access to health-related and
personal information to only those people who need to know it in order to
provide customer service and support. These people are trained on the
importance of safeguarding this information and must comply with our procedures
and applicable laws.
Electronic access to private health information is also restricted to plan
members and covered healthcare providers, and is only accessible after entering
the authorized user ID and password. The information displayed by our
system is secured and encrypted during transmission to ensure privacy during
use. Furthermore, the information you provide or that is displayed will
never be used for purposes not related to treatment, payment, or other
healthcare operations.
