Which pharmacies are participants on our prescription program through Medco?
The City of Mesa's prescription benefits are administered by
Medco, Inc. Participating pharmacies
include but not limited to: Walgreen's, Target, Fry's, Safeway, Albertson's,
Wal-Mart, Basha's, Costco, CVS, and Osco Drug. For a complete listing of
participating pharmacies, contact Medco at 1-800-711-0917, or visit
www.medcohealth.com. (Registration with valid
e-mail address is required to view participating pharmacies for your area.)
Is there a list of in-network dentists on the City's Dental Plans?
No, there is no list of in-network dentists. Members can visit the dentist
of their choice.
Can I get a replacement health insurance card?
Medical/Prescription Cards -
Members enrolled in the City of Mesa Medical Plan were sent new ID cards in
the mail from Medco the first week of April 2008. These cards combine your
medical and prescription drug benefit information.
Two cards were sent to each member's mailing
address. If you need additional cards, you can
request them from Medco at 1-800-711-0917. The
cards will have only the insured's name, not each
individual dependent's name.
Since this card combines information from both
Medco, our prescription drug administrator, and the
City of Mesa Medical Plan (Mesa Choice Plan), please
refer to this guide on your new card:

Dental Cards - Only new hires and members
who made changes to their dental plans during Open
Enrollment received new cards for 2008. If you
need a replacement card, please contact Employee
Benefits to receive a non-personalized dental card.
Vision Cards -Vision cards were not issued
by Spectera. To view your benefits, please
logon at Spectera* to view your member
benefits.
*When logging in as a member on
www.spectera.com,
your member ID made up of 4 leading zeros followed
by your 5-digit employee ID.
HealthSmart Cards
- If you and/or your
dependents are living outside of Arizona, you will
not receive new HealthSmart cards.
Out-of-state plan members were sent new
prescription/medical cards from Medco.
If out-of-state members are visiting Arizona,
please remember to use a BCBS of Arizona PPO
provider for your best benefit. Members will want to
present their combine/prescription/medical cards
from Medco to their Arizona healthcare provider .
Replacement HealthSmart cards may be requested
from the Benefits Office.
What benefits can I enroll in
or change during open enrollment?
During open enrollment, you have the opportunity to enroll in or make
changes to the medical, dental, vision, short term disability, flexible spending
accounts, and supplemental life insurance.
Is Open Enrollment the only time I
can make changes to my healthcare benefits?
No. You can make changes to your benefits when you experience a qualifying
change in family status. This is considered a qualifying event, which
allows you to make certain changes to your benefits during the year.
What qualifies as a change
in family status?
There are certain situations that may qualify as a family status change.
Some of the more common are: marriage, divorce, birth or adoption of child, or
involuntary loss of other healthcare coverage. If you experience a family status
change, you only have 31 days from the event date to enroll the dependent(s) to
your insurance plans.
What happens if I forget to add
my dependent(s) to my insurance within the 31-day timeframe?
If you fail to enroll your dependent(s) within the 31-day timeframe, you will
have to wait until the following open enrollment period to do so. No exceptions
will be made.
Can I enroll in family coverage for
the medical insurance, but only single coverage for dental insurance?
Yes. You can mix and match your levels of coverage. However, you and all your
covered eligible dependents must be enrolled in the same medical, dental, and
vision plans.
Why does my paycheck indicate I am
paying much more for my benefits than what I should be paying?
Your paycheck indicates what you as an employee pay for the insurance and what
the City of Mesa as an employer pays for the insurance. The columns stating
Before-Tax Deductions and After-Tax Deductions list the amounts the employee
pays. The column stating Employer Paid Benefits lists the amounts the City of
mesa is contributing toward your premiums.
Do I contact you for information on
the deferred compensation plans?
No. Any questions regarding deferred compensation should be directed to the
Payroll department at 644-2349.
Where can I get information on my
current vacation and sick days?
Payroll (644-2349) can provide this and any information regarding your paycheck
to you.
Currently, I am legally separated
from my spouse. Can I drop him/her from my insurance?
Yes. You must provide Employee Benefits with a copy of the legal separation
document, along with the appropriate forms to drop your spouse.
