Costco No Longer In-Network Vision Provider for
Glasses and Contacts on May 1
Costco store locations
will be out-of-network for optical materials
effective May 1, 2008. This contract affects
all Costco locations throughout the country and is
based on a business model change made by Costco.
The doctors at Costco will remain in-network, but
purchases of glasses and contacts at these locations
will no longer be considered in-network after May 1,
2008. In order to best serve our members, a
transition period will allow Spectera members to
continue to receive network benefits on glasses or
contacts purchased at Costco from May 1, 2008
through July 31, 2008.
Members who purchase materials (glasses or contacts)
at Costco from May 1 through July 31, 2008, will
need to pay in full at the time of purchase and then
submit a Costco claim for reimbursement. (See
below for claim form.) For member convenience,
Costco will have the custom claim form available at
their stores during this transition period. A Costco
claim form can also be downloaded from Spectera’s
website, www.Spectera.com, or requested from
the Customer Service Department at 1-800-638-3120.
Members submitting the Costco claim form with their
receipt will be reimbursed on an in-network level
for purchases during this transition period.
Starting August 1, 2008, materials purchased at
Costco stores will be handled as an out-of-network
benefit and reimbursed based upon our out-of-network
reimbursement schedule. Please keep in mind that if
vision members choose to purchase eyewear from a
Costco location, they must have a valid Costco
membership card.
The good news is that
Spectera vision members will continue to receive
in-network benefits for comprehensive exams that are
performed in the optical departments at Costco after
the transition period ends. Please note that
Spectera vision members can receive an eye exam from
a participating doctor at Costco even if the member
does not have a Costco membership.
Beginning August 1, 2008 all purchases made at
Costco Optical Centers will be reimbursed based upon
our out-of-network reimbursement schedule.
Reimbursement will be applicable only to those
eligible for benefits.
Costco Claim Form for Reimbursement
When submitting receipts for
reimbursement for Costco (or any out-of-network
provider), you will need the following on each
page of your receipt:
· Member’s name and ID number*
· Patient’s name and date of birth
*NOTE: Your Spectera member ID is a 9-digit
number consisting of your employee number with 4 or
five leading zeros, depending on your ID. Ex.
000002222 or 000019999.
